A document which authorizes someone else to act as your agent or attorney-in-fact. The document is usually recorded in the Clerk of Court’s office.
A form of power of attorney which does not end in the event of incapacity.
A document in which you authorize an agent to make health care decisions for you if you are unable to make those decisions.
A document which applies to only two medical situations at the end of life (terminal with death expected within a reasonably short time if life sustaining measures are not taken or persistent vegetative state).
